Gestational Diabetes is one which develops or is deducted for first time during pregnancy and it is mainly because of increase in insulin which happens during pregnancy and it classically disappears after pregnancy. But many of these women are at high risk for developing Gestational Diabetes later after pregnancy. So the care for these women with Gestational Diabetes does not abruptly end with the delivery of the baby. After the delivery the immediate postpartum period insulin resistance resolved quickly and so does the need for insulin. If one had been in diet control for the Gestational Diabetes we stop monitoring glucose levels if the postpartum period, if she were on`` insulin we recheck her sugars on the day of discharge probably 3rd day after postpartum so it is important to recall these women at 6 weeks postpartum and perform a 75 grams glucose tolerance test, ideally this test is performed between 6-12 weeks postpartum because it gives us the opportunity to help and prevent the development of diabetes later in women. So what values are we looking for in this 75 grams glucose test the fasting less than 100 is considered normal, 100- 125 is considered as imp-ad fasting glucose and 126 and above is provisional diagnosis of diabetes. Two hours after 75 grams glucose the ideal value is less than 140 which is considered to be normal, between 140- 199 is considered as im-pad glucose levels and 200 milligrams and above is a provisional diagnosis of diabetes. So these women have to be council-ed by a obstetrician and dietitian to help them control and manage their diet, physical activities and weight management. If required the pharmacological interventions need to be given, but remember it is not just the health professionals who have to council these women a lot of familiar support is required and need to be extended for these women in the postpartum period to help them find time to do their daily physical workouts to help them prevent being a diabetic later.
